Only what you could noticeInternal refactors and work still behind a gate are not entries. A change belongs here when it altered what someone can do.
Dated by release, not by decisionThe date is when the change reached people, not when it was approved or merged.
Named owner per entryEvery entry carries the person accountable for it being true, so a wrong entry has somewhere to go.
No silent editsA corrected entry says it was corrected. Rewriting history is how a changelog stops being evidence.
